LIGHTHOUSE is on a mission to transform the quality of life for people living with chronic conditions. Central to this mission are our team of distributed nurses, counselors, dietitians and counselors that help our patients along their health journey. We are looking for a product manager to lead our BEACON Platform which matches our Care Team Members (our “Beacons”) to Care Opportunities for our patients. Our BEACONS log in each day to complete care activities for their patients, call patients, follow up on open tasks and meet care engagement guidelines.
In this role you will own the BEACON/patient experience, from “care recipe” implementation to care quality tracking.
You will be successful in this job if you have:
- Experience leading the development and management of client service experiences
- A passion for developing tools to enhance the quality of care delivered to patients
- Interest in creating a federated-care management platform similar to how Uber manages their drivers
